Sample registration form

You can make a simple registration form on Google Docs with all kinds of fields. Then click on the "More actions" button in Google Docs and get the embed code to paste the form into the web site.

Data collected will end up in your Google spreadsheet.  Here a zillion help sites and videos that show you how to do this if you search on "how to make a google form."

The one trick for embedding here on the web site is to first click on the "disable rich-text" link to turn off the rich text toolbars above the text area, and then paste in your embed code.
